Step 1: Confirm basic fit and safety
The website worksheet and complimentary discovery conversation are not medical assessments. They confirm that the patient can attend in person, understands the direct-pay structure, is not seeking emergency care, and has expectations that match the program’s limits.
No Connecticut-residency question is required. The practical requirement is the ability to attend appointments at 55 South Road, Suite 120, Farmington, Connecticut.
Step 2: Complete the physician evaluation
The extended visit with Dr. Patel focuses on the cognitive concern or prevention goal, neurologic and medical history, medications and supplements, sleep, mood, pain, headache, vascular and metabolic risk, family history, completed workup, daily function, and the patient’s priorities.
This is consultative care. Existing primary-care clinicians, neurologists, therapists, sleep clinicians, and other specialists remain central to the patient’s care.
Step 3: Use selective testing—not a fixed panel
Testing is chosen only when a result can plausibly change diagnosis, counseling, treatment, referral, or monitoring. Some patients may need laboratory studies, cognitive testing, imaging review, or another specialist. Others may need less testing or a different clinical pathway first.
Comprehensive care means asking the right questions and interpreting the answers in context. It does not mean ordering every available biomarker, scan, or commercial panel.
Step 4: Receive synthesis and a written plan
The final phase organizes the symptoms, history, risks, medications, and results into priorities. The written plan explains what appears most important, what to address first, what may be monitored, and which clinicians should stay involved.
With written authorization, relevant findings and recommendations can be organized for the referring or existing care team.
What exactly does the $4,500 fee cover?
The one-time Brain Health Evaluation & Plan fee covers three physician visits, the extended neurologic evaluation, review of relevant records and results, clinical testing strategy, synthesis, and the written care plan. A Good Faith Estimate is provided before scheduling.
Outside services are not included unless stated in writing. The optional $5,000 annual Physician Membership is considered only after Door A is complete; it is not required to begin the evaluation. The comprehensive integrative program is currently closed to enrollment.
Who may need a different first step?
The program is not appropriate for sudden neurologic symptoms, rapidly worsening cognition, active mental health crisis, unsafe behavior, advanced dementia requiring a different level of care, or expectations of guaranteed cure, reversal, or prevention. When another pathway is safer or more appropriate, that recommendation comes first.
