Clinical posture

Selective testing, interpreted in neurologic context.

Comprehensive does not mean indiscriminate. A conditional test should be able to change diagnosis, counseling, treatment, referral, or monitoring. If it cannot, it is not ordered.

When the evaluation reveals a condition that needs conventional diagnostic care, the program pauses or ends and the patient is directed to the appropriate pathway. A diagnostic problem is not converted into a program enrollment.

Appropriate referrals

Patients this may fit.

Subjective cognitive concerns or brain fog after an appropriate standard workup
Persistent, clinically stable cognitive symptoms after concussion or traumatic brain injury
Headache, sleep, medication, vascular, or metabolic factors that may be clouding cognition
Family history of dementia with realistic expectations about modifiable risk
A patient who needs more time for synthesis than a routine visit allows
Patients able to attend the Farmington office who understand the direct-pay structure

Different first step

Rapidly progressive cognitive decline or new functional loss
Acute neurologic deficits, suspected stroke, seizure, or another emergency
Moderate or advanced dementia requiring a different level of care
Active psychiatric crisis, suicidality, or unsafe behavior
Untreated severe sleep apnea or severely uncontrolled medical risk
A request for guaranteed reversal, cure, or prevention

What comes back to you.

With the patient's written authorization, the referring clinician receives enough information to understand what was found, what was recommended, and what may affect standard care.

No promised outcome and no testimonial presented as typical evidence.
No broad exotic panel used as routine screening.
No routine APOE baseline; genetic testing is conditional and counseled first.
No manufactured protocol when the evaluation does not support one.

Financial expectation

Please set it before referring.

The Brain Health Evaluation & Plan is a $4,500 direct-pay service. It includes three physician visits, clinically selected testing and imaging review, synthesis, and a written plan. Outside laboratory, imaging, supplement, device, and specialist costs are separate.

How to refer

Start with a brief clinical note and a phone call.

Include the cognitive concern, relevant history, completed workup, and what you would like clarified. The patient then completes a fit and safety screen before any enrollment discussion.
